INFORMATION FOR PATIENTS OF THE 10th MILITARY RESEARCH HOSPITAL AND POLYCLINIC, INDEPENDENT PUBLIC HEALTHCARE CENTRE IN BYDGOSZCZ
When arriving for a hospital stay, the Patient must bring:
his/her ID card,
a monthly report for persons insured in the national insurance system (ZUS RMUA) or a document confirming insurance as a member of an insured person’s family (ZUS ZCNA) or another document confirming payment of health insurance contributions;
a valid hospital referral.
Patients are admitted on the ground floor of the main hospital building, in Admissions, where the registration clerks create a medical history file based on submitted documents. Next to the Admissions, there is a cloakroom with deposit boxes where Patients may change their clothes and leave their belongings (except for valuables, such as money or jewellery).
The Patient is informed of the discharge date the day before discharge. If the Patient is employed, he/she needs to notify the medical secretary or the head nurse that a doctor’s certificate is needed, and provide his/her ID card and insurance card with a valid stamp and taxpayer identification number of the employer.
On the discharge day, the Patient notifies the maintenance worker of a given ward that he/she needs to collect his/her deposited belongings.
Monday to Saturday, 1.30pm – 6.00pm
Sunday and holidays, 10.00am – 6.00pm
Anaesthesiology and Intensive Care Clinical Ward:
Monday to Sunday, 2.00pm – 5.00pm
Depending on the patient’s condition, visits may be limited or, if reasonable, may take place at other times, subject to manager’s approval (Head Doctor).
